• Form 2 – New Student Registration Form
• Form 3 – English Preparatory Form (if applicable for International Students Only)
• Form 4 – Financial Guarantee Form (for Malaysian)
• Form 5 – Personal Data Protection Notice
• Original scan of Health Assessment Form (Verified by Medical Officer)
